- Any circulatory conditions (problems with blood flow, including strokes, high blood pressure, and cholesterol)?
2. Has anybody listed on the policy been prescribed medication, or received treatment or attended a medical practitioner’s surgery in the last 2 years?
3. Has anybody listed on the policy ever been diagnosed with or treated for any psychological conditions such as stress, anxiety, depression?
4. Has anybody listed on the policy ever been diagnosed with or treated for any psychiatric condition such as eating disorders, drug or alcohol abuse or mental instability?
5. Has anybody listed on the policy attended a hospital or clinic as an outpatient or inpatient in the last two years?
6. Have you or has anyone in your party been currently put on a waiting list for treatment or investigation?
7. Are you or is anyone in your party suffering from an undiagnosed medical condition or any undiagnosed symptoms?
8. Have you or anybody in your party received a terminal prognosis?
